You Choose Labs LLC, (YCL) is an independent mobile phlebotomy service provider. We provide in home mobile blood draws on a concierge basis to our clients in the privacy of their home, work space, or any place that our clients prefer to have their blood drawn. We service Metro Atlanta, GA and surrounding areas up to 30 miles (round trip) from our business location of, 11340 Lakefield Dr. Johns Creek, GA 30097. We do not service clients at our business address as our services are provided on a mobile basis and we travel to our clients. Our fees are NOT covered by insurance and will require payment at time of booking to secure appointment.

You Choose Labs LLC is responsible for: travel to client within 30 miles (round trip) of business office location, specimen collection, preparation, processing, handling, and delivery to laboratory for testing. YCL does not collect any specimen without a test request from a healthcare provider. Your lab request can be faxed to our secure fax server at 1-833-471-3455 or emailed to avia@youchooselabs.com.

You Choose Labs LLC does not give advice, treat, or diagnose.

You Choose Labs LLC, (YCL) handles the specimen from the time of collection until delivery to the laboratory or shipping facility. At no time will specimens be given to client for shipping, handling, or delivery. YCL is not responsible for any errors that may occur once specimen is delivered to the lab or shipping facility.

Payment of invoice is due upon receipt of invoice up to 9pm the night before scheduled appointment. If payment of invoice is not paid by 9pm the evening before the lab appointment, the appointment will be cancelled. Payment of invoice confirms appointment. Invoices will be emailed upon receipt and review of lab order. Our service fee starts at $75. Requests for services 30 – 75 miles (round trip) will incur an additional $25 travel fee and a $40 fee with travel beyond 76 – 100 miles.

Once lab order is received and reviewed, client will receive an invoice requesting payment to secure an appointment. Appointment for lab services is tentative until payment of invoice is received. Only then is appointment confirmed. Once invoice is paid, an email or text will be sent confirming appointment.

If we arrive at the appointment and we are unable to collect the specimen due to circumstances beyond our control, i.e. patient refuses, client not available at confirmed time, client hasn’t followed preparation instructions as per testing requirements or physician, the client will be responsible for the full charges of the collection. There will be no refunds except in extenuating circumstances.

You Choose Labs LLC, does not bill insurance companies for our fees. Client may submit invoice for our services to their insurance company in order to obtain reimbursement. However, we do not guarantee payment. Client’s lab testing will be billed to their insurance provider by the testing facility. Lab results for testing requested by client’s medical provider, will be sent to the ordering physician only. We encourage our clients to set up a patient portal with either Quest Diagnostics or LabCorp in order to receive a copy of their test results as well. Any charges related to lab testing, is the responsibility of the client and must be handled with the billing department of the testing laboratory as testing is billed to the insurance company and we do not handle billing. We are not responsible for testing not covered by your insurance company. You Choose Labs LLC does not file insurance claims. You Choose Labs LLC is not associated with any doctor’s office, insurance company, or medical facility. Under no circumstance will You Choose Labs LLC divulge any client information to any unauthorized personnel. You Choose Labs LLC is fully insured and HIPAA compliant, we respect the privacy of each of our clients.
