Do you offer COVID testing for active infection?

No.

Will the request ordered by my physician be covered by my insurance?

You Choose Labs does not bill insurance or handle claims. Lab testing is billed by the testing facility. Lab billing questions must be directed to the billing department of the testing facility.

Which laboratory are test samples delivered to?

Client test samples are taken to Quest Diagnostics, Labcorp, UPS, or FedEx.

You Choose Labs, LLC handles client specimen from time of collection until delivery to lab or shipping facility.

*At no time will additional tubes be drawn and given to client for delivery or other means.*

Will I receive a copy of my results?

We recommend our clients to create a patient portal with Quest Diagnostics or Labcorp. Once your patient portal is created, whenever labs are tested by the facility, results will be automatically sent to the client once they’re available.

*For Self-Ordered tests, results are automatically be emailed to client

Can I request same day services?

Yes, however, there will be an additional $50 same day service fee assessed to accommodate this request.

Do you offer STAT service?

Yes, however, there is an additional $100 service fee included on the invoice. These requests are delivered to the immediately. The request will be confirmed with the ordering physician and testing facility as there are only certain tests run STAT as well as when they’re run.

What information is needed on the lab order from my doctor?

1. Quest Diagnostics or LabCorp account number

2. Ordering physician’s name, NPI number, office address, telephone number, and fax number

3. Patient’s name, date of birth, telephone number, tests requested, diagnosis codes, and copy of patient’s primary and secondary insurance card if applicable.

Can services be done at an assisted living facility?

Yes, we’ll need a complete lab order from the ordering physician to include:

1. Ordering physician’s name, NPI number, address, office phone & fax number
2. Patient’s name, date of birth, insurance cards, diagnosis codes, and tests requested.

We’ll also need the name and telephone number of the person who coordinates care for patient needing labs.

I have more than one lab order, can they all be done at the same time?

Yes, there is a $5 fee assessed for each additional lab order.

I don't feel comfortable with someone inside my home, can the service be performed in my garage?

Yes, we’ll require a small table for set up and a chair for the patient who’s receiving services. If the patient is prone to passing out, we may not be able to perform this service without being inside the home unless there is someplace for the client to recline or lie down.

Do you draw on infants?

We do specimen collections for patients from 1yr and up.

NO, insurance does NOT cover our service fee which starts at $85. The invoice may be submitted to your insurance provider, however, we DO NOT guarantee that our service fees will be reimbursed.
No, our services are offered on a concierge basis so we travel to our clients in order to perform services.
Our service fee starts at $85 and up depending on client location up to 10 miles of our business location. Travel beyond 20 miles round trip will incur a $0.57/ mile fee.

Being that our services are an out of pocket expense, we’ll also need the email address, name and telephone number for the person that will be paying for the service.

We do specimen collections for patients from 2yrs and up.

Typically, you visit with your physician, your physician requests blood work and you’re given a lab order. You’ll either make an appointment, or walk-in to a Patient Service Center like Quest Diagnostics or Labcorp to have your blood drawn. Your sample is collected, processed, and delivered to the testing facility to be tested. Once your results are final, they are then forwarded to your ordering physician.

With our service, we eliminate the need for you to make that lab trip! Once you receive your order from your doctor, you can either email it, fax, or have your doctor’s office forward it to us directly along with a copy of the insurance card. You can book your appointment online or call to book your appointment. Once we have received your documents and an appointment is booked, we will contact you to confirm the receipt of your lab order, appointment details, instructions, and an invoice will be emailed to secure your appointment slot. On the day of your appointment, your certified phlebotomy tech. will arrive to collect, process, and deliver your samples to the testing facility (Quest Diagnostics or Labcorp). The laboratory will test your samples and forward the results to your ordering physician as well as bill the insurance company for running the tests.

Patients who do not have insurance or would like to order tests on themselves can do so by accessing our Direct Access link on our Home or Services page.
